Sam wakes up from a black-veined, fog-like nightmare after the events of "Don't Call Me Shurley," 11x20. He heads to the library for a nightcap, but instead, he finds Dean.
"Sam clears his throat before walking in, giving Dean time to school his expression, or drop the amulet, whatever he needs to do. But he doesn’t do any of those things. He looks up at Sam with the same gentle eyes he was using to search every inch of the necklace, and Sam smiles at him, can’t help it. They’ve been a unit for almost three decades, save the years Sam was in college, and the fact that Dean can still look at him like that, like he’s the only motion in a still world, the only color in monochrome-- it gets to Sam."

"And so often, I get up in the morning and Dean’s right in Sammy’s cot with him, arms wrapped around him tight. He’s never stopped being protective of Sammy, ever since that night. But now I’m realizing, Jesus, they must be just about keeping each other alive. I sure as hell haven’t been giving them that. If they grow up normal at all, if they thrive, it’s no thanks to me.”
John's journal has some particularly personal entries that Sam and Dean had respectfully decided never to read. Until one day they decide to read them. Lots of feelings, then eventual healing sexiness, ensue.

Summary
Dad would push Sam to Dean’s arms every time and Dean would change his diaper without a word of complaint. It felt familiar back then. It reminded him of Mum.
At that age, Dean wanted to do everything Mum did for Sammy. It hurt him whenever he couldn’t do something. He had forgotten about it. The helplessness he felt when he couldn’t provide for Sam.
Looking back now, it was such a stupid feeling. Obviously, Dean couldn’t make milk for Sam. He wasn’t a girl. He was also too young to be able to do it anyway. God, Dean couldn’t be more thankful that nobody else knew about it. Sam didn’t remember and Dad never knew. Dean never let Dad see. He could feel Dad’s eyes following Dean around. He knew Dad saw someone else there. It made Dean just the tiniest bit happy to be able to make Dad smile but he couldn’t let Dad see how he failed at the one thing Mum could always do; make milk.
OR
Dean never gets over wanting to breastfeed Sam and being unable to do it. It escalates when Sam finds out.

It's been a long time since things were this bad between them. Since the last time Dean decided he didn't give two shits about what Sam said. It's bad enough to make Sam cringe now, wondering how he'll ever get on Dean's good side again. If that's even possible. How does one say 'Sorry, I didn't rescue you from Purgatory'?
You don't, he thinks to himself.
Finding an empty house in the woods where hikers have been disappearing might not be the best place to finally clear the air, but it seems as if they won't have much choice once Sam gets taken and Dean is faced with just how bad Sam is dealing with their current conflict.
