Deku bought Katsuki enough air to live through the sludge villain attack. Try as he might to deny it, Katsuki feels like he owes Deku for that, and patiently waits for Deku to tell him it's time to pay up. Eventually, Katsuki stops waiting for the nerd to bring it up and takes matters into his own hands. His own mouth, whatever.
