Baelor makes the decision to wed Maekar to him after his sons and wife die during the Great Spring Sickness and Baelor assumes the throne. It would be easier if they had spoken since the tournament at Ashford. It would be easier if Maekar had not been in love with him since they were boys.

Aerion finally meets his gaze. The flames dance in his eyes, and Dunk thinks that he sees something. But he can’t delude himself any longer. There’s no regret or guilt. There's no love or even anger.
Aerion’s eyes are hollow amethyst.

Nothing at all.
At Ashford, Dunk learned the hard way to stay away from beautiful omegas with vicious hearts. Five years later, it turns out he’s learnt nothing at all.

House Targaryen consists mainly of low-level doms, subs and switches. Their needs are easily met. They don’t need to go into subspace or domspace often. However, Aerion, son of Maekar, is a high-level sub. None of his family members are able to give him the care that he needs, so he often acts out and is generally a nightmare to deal with. They contact Duncan Pennytree, a high-level dom who can hopefully bring Aerion to heel. But Aerion absolutely isn’t going to make it easy for him.
