“Come on,” Dottore wheedles. “It’s going to get cold if we leave it any longer.”
“Just eat first,” Pantalone says and yawns widely, deliberately, to prove a point. Then he pulls the comforter back up and snuggles back into his pillow.
And the comforter is immediately yanked away as Dottore rips it off his sleeping body.
Pantalone and Dottore are partners in crime, always up to no good. It's just like he remembers! Like always. Nothing changes.

Summary
Somewhere in his coughing fit he fell to his knees, lacking the strength to remain standing. Pantalone spits out blood before anything else; the flowers soon follow. The pain in his lungs and throat override every other sense. It feels like someone is pushing down on his head, as his eyes see blurry, and his chest raises up and down rapidly.
As soon as the coughing noises lessen, Dottore’s voice is there to fill in. “I will perform the surgery on you, Feofan,” he declares, as if it’s his choice to make.
Pantalone has yet another illness, and Dottore wishes to help before even knowing what it is. Pantalone takes issue with that.

Watching him struggle, Zandik starts running, and finally they collide—disastrously, cosmically. Zandik swoops him up in his arms, careful not to squeeze him. Their bodies coil around each other, locked in an embrace impossibly tight but gentle.
Feofan laughs, a true laugh. It resounds in an echo in his lungs, a sweet melody bouncing off those ashy walls.
“Zandik!” he says, delighted.
Or, Feofan is dying.
