He even, intellectually, thinks he understands his appeal, he is sterile and so cannot leave them with an unwanted pregnancy, a painful labor and yet another mouth that will need feeding. How does that translate into his appeal to Jaskier?
Geralt doesn't 'get' humans - but he wants to. Wants to be included and thought of and even in his deepest fantasies liked. So if he does things he doesn't particularly enjoy for Jaskier's benefit. Does it matter? If it let's him keeps his friend?

Anal sex is no commonplace act in any part of the Continent and Geralt has never been with a man. Without anybody to teach him differently, he can only believe what he overhears in taverns and casual conversation -which is that anal intercourse is uncomfortable at best, intensely painful at worst.
