As the oldest, Raph had many privileges his younger siblings didn't have.
He could stay up longer, he could be in charge, he could try new things sooner-
He didn't get sick.
A simple fact. He was the oldest, the biggest, the strongest. He doesn't get sick. He can't afford to, he needs to take care of his brothers- but he doesn't need to worry about it because he's the oldest which means he doesn't. Get. Sick-
"'Doesn't get sick' he said." Meanwhile, the soft shell grumbled, using his wrist gauntlet to take his temperature again, frowning with disapproval. "Congratulations, Raphael. You have a fever as high as any warm-blooded being's body temperature."

Summary
It was a simple question: What did Raph mean when he said 'April's the big brother now?'
Let's take a journey back to when the turtles and April were younger, when children are still figuring out their place in the world, and maybe the first time Raph got weird.
This is a direct sequel to "Anger is the Color of Your Energy," so I'd advise reading that before moving on to this as there will be references to things that happened in that story. But do what you want, I'm not in charge of you.
See the end notes for some of my thoughts on how Raph "getting weird" hasn't been explored as thoroughly in the fandom.

Doesn’t stop his entire body from twitching forward, alarm spiking like lightning through his limbs. Mikey rushes forward to help Raph back onto his feet with impossibly gentle hands.
He doesn’t think he's ever had to worry about Raph before. Not like this.

Summary
Raph stepped out of Donnie's makeshift lab, finally getting his younger brother to sleep after spending all day tinkering and screaming about his next new miracle. He couldn't help but sigh and finally slump his shoulders, feeling tired after having to convince all his younger brothers to sleep, they all had bounds of energy that he wish he could have.
Though really, its energy he should have had, he was 10 after all, he was meant to be carefree and stomping around, desperate to release built up of energy and creativity he held but instead, he wanted nothing more than to lay down and just relax all day, like his pops.
His eyes widened, his pops, he needs to check up on him.

Michelangelo believes Donnie is a good person, in spite of the impressions his brother might leave sometime. Almost everything the softshell does is to help and protect his family.
Not even discovering his brother has a terrifying new power does anything to sway this feeling, even if it leaves him a bit concerned.

As a good Leader™ Leonardo knows he should face the issue and try to get his twin to talk about it, but he's a bit surprised to find out what the root of the issue actually is.

“There are days where Raphael really, really wishes he was a middle child rather than the eldest brother. Perhaps in another life, another time, his wish is granted and he lives happily without having to deal with his brothers having temper tantrums over the slightest inconveniences. But that reality isn’t his, so he is stuck having to master the art of “getting my little brothers to calm down before they start biting”.
Because his brothers, in all their glory, are overdramatic. In Donnie’s case, full on theatrics. And Mikey only needs to get a slight hint that he could start acting up and he’s suddenly in his shell for the next three days because you didn’t give him a milkshake.
Like Raph said. Maybe if he wishes hard enough he can pretend he is a middle child. Leave Leo to take care of this and see how he feels.”~
Raph, being the eldest sibling, is well-versed in each of his brothers’ behaviours when they lose their temper, but it doesn’t make dealing with them any easier.
TLDR: Each of the turtles’ behavioural habits when they lose their temper. Mostly fun stuff about turtle instincts.
