Akutagawa’s response comes out in shaky sobs. “I can’t, Jinko—you don’t understand—my body—”
“Is it because you’re sick? I’ll be careful, I promise. I just want you to get better, Akutagawa."
The other man shivered at the usage of his name, and didn’t say anything. His wrists were small and bony, and Atsushi could wrap his fingers all the way around them. He stared down at Akutagawa’s hands; his carefully manicured fingers were trembling, the callouses on his palms seeming almost out of place against his porcelain flesh. Without thinking, he lifted Akutagawa’s hand, and kissed his knuckles gently.
Akutagawa stared at him with disbelief, his face still a pretty pink, lips parted. Atsushi met his hazy grey eyes.
“It won’t ever go away if you don’t let someone help,” Atsushi said gently, “Please let me help, baby.”
i.e. atsushi fucks aku so he doesn't die

As far as society at large is concerned, he's already Kalim's bitch, and surrendering himself to both of their instincts would make it true. Like this, instead, he can hold on to the knowledge that when people see the alpha heir making eyes at his omega attendant, whatever they're assuming, they're wrong. Even if the two of them are the only ones who know, and even if Jamil is the only one who cares, those people are wrong.

One would expect that after becoming a pseudo immortal entity and being bound to a eldritch monstrosity for life, banal things like secondary gender characteristics would become irrelevant. Specially those related to the estrus periods that reigned his life during his miserable teenage years.
Tim was in his 30s, he expected that living a life as a killer pawn would spare him the uncomfortable reminder that he used to be a –somewhat– prickly underfed omega. He was pretty sure that a diet of cigarettes and beer should have killed any estrus cycle by now, but alas here he was again: miserable, sweaty and about to die of an aneurysm.
Tim was undeniably an omega.
